1. Call or send a fax
Call (305) 465-9379, fax the referral to (305) 203-4672, email info@pmcen.com or complete the online form.
2. We evaluate the patient in 24 to 48 hours
Our intake team contacts the patient, verifies insurance and schedules the initial assessment.
3. We coordinate care with you
With the patient consent, we keep you informed and coordinate the transition back to your care.

Monday – Friday
Intensive Outpatient Program (IOP): includes our Substance Use Disorders treatment services. Three-hour shifts; choose the one that fits your day.
